This is something I see very often in clinic. A patient comes in worried about acne, but when you look closely, the acne is not very severe. What really stands out are the marks, scabs, and healing wounds. And almost always, when you gently ask, they say, “Yes doctor, I keep touching and picking it.” That is acne excoriée. It simply means acne that has been picked again and again, until the skin gets damaged more than the acne itself.

It usually starts very innocently. A small pimple comes up. You feel like removing it. You touch it, squeeze it and maybe pick at it, thinking it will heal faster. But the opposite happens. The pimple becomes a wound, then a scab, then a dark mark. And because it’s now more visible, you notice it more and your hand goes back again. Without realizing it, a cycle begins, pimple, picking, wound, mark, and then more picking.

And you are not alone in this. Around 2–3 out of every 100 people go through this habit of repeatedly picking their skin at some point. Many don’t even realize when it slowly becomes a pattern.

On the skin, you may not see typical acne anymore. Instead, you see small cuts, scabs, red areas, and dark spots. These are usually on areas you can easily reach like the cheeks, forehead, and chin. Sometimes, people even start picking at very tiny bumps or even normal skin without realizing it.

There is also a very human side to this. Many people pick more when they are stressed, anxious, overthinking, or even just sitting idle. Some do it in front of the mirror. Some don’t even notice when their hand reaches their face. At that point, it becomes less about the pimple and more about the habit.

From a skin point of view, every time you pick, you are damaging the skin’s natural barrier. This slows down healing and increases redness and irritation. It also triggers pigment cells, which is why those dark marks take so long to fade. If this keeps happening, it can go deeper and lead to permanent scars.

That’s why in acne excoriée, the main problem is not the acne, it is the picking.

Treatment is simple, but not always easy. Your skincare should be gentle and minimal, a mild face wash, a good moisturizer, and sunscreen are enough. Avoid harsh scrubs and trying too many products at once. But the real change comes from controlling the habit. Keeping nails short, avoiding repeated mirror checking, using pimple patches, and keeping your hands busy can all help. The most important step is simply becoming aware of when you tend to pick during stress, boredom, or overthinking.

If you feel like you just can’t stop, that’s okay. It’s more common than you think, and sometimes you may need a little extra help to break the cycle.

Your skin actually knows how to heal. It doesn’t need you to fix every pimple. It just needs you to leave it alone long enough to recover. And honestly, that’s the hardest part but also the most important one.
